A Patient-Centered Medical Home (PCMH) is not an object, but rather an approach to provide comprehensive health care. It’s called a “home” because we want our office to be the first place you think of for all of your medical care needs. Our goal is to make it easy and comfortable to get the care you need in a way that works best for you. We are here as primary care providers, to facilitate a personal partnership with you and your family to provide you with the best quality, comprehensive and team-based primary care.
A Patient-Centered Medical Home (PCMH) is a model of primary health care that aims to ensure timely access to comprehensive, patient-centered care delivered by teams of primary care providers and their staff, who coordinate all aspects of a patient’s health care needs. We are committed to providing accessible, quality, evidence-based healthcare to everyone. Your health is our main priority, and that’s why we put you at the center of your care. Our providers are highly qualified, and licensed in the following areas: family practice, pediatrics, obstetrics, gynecology, and behavioral health. Working in a team-based approach, we offer evidence-based whole-person care, preventive/wellness care, care for diabetes and other chronic conditions, care when you are sick, and education and self-management support.
