The Hart Supported Living program provides grant funding for Kentucky residents who have a mental and/or physical impairment that substantially limits a major life activity such as walking, talking, seeing, hearing, caring for oneself, or working.
- Applications are distributed to interested persons beginning the first of January each year. Completed applications must be submitted by April 1st each year.
- Applications are reviewed at the state level and are awarded based upon intensity of need and available funding.
- Examples of supports which can be requested are home or vehicle modification specific to the physical impairment, adaptive and therapeutic equipment, homemaker services, transportation, personal attendant, etc.
